Quebec's public health insurance plan (RAMQ) is one of the first matters to sort out after you arrive in Montreal. Unlike Ontario or British Columbia, Quebec runs its own system with its own waiting periods, forms, and rules. Many newcomers discover too late that there is a three-month waiting period, leaving them without medical coverage in an emergency.
What is RAMQ?
The Régie de l'assurance maladie du Québec (RAMQ) is the public agency that manages health insurance for all Quebec residents. Your health insurance card (carte seule) gives you free access to medically necessary services: doctor visits, hospital stays, surgeries, lab tests, and radiology. RAMQ also covers part of prescription drug costs for people not covered by a private insurance plan.
Important: RAMQ does not cover routine dental care, private physiotherapy, eyeglasses, or care received outside Canada. This is why a supplementary private insurance plan is often still necessary.
Who is eligible for RAMQ?
You can register if you fall into one of these categories:
• Canadian citizen or permanent resident settled in Quebec;
• Temporary worker with a valid work permit of at least 6 months;
• Temporary resident with a valid permit of at least 12 months, under certain conditions;
• International student from a bilateral-agreement country (France, Belgium, Denmark, Norway, Sweden, Finland, Iceland, Romania);
• Accepted asylum seeker.
Tourists, visitors, and people without immigration status are not eligible. International students from all other countries must enroll in their school's mandatory student health plan.
The three-month waiting period
This is the most commonly misunderstood point. RAMQ applies a waiting period: coverage begins on the first day of the third calendar month following your registration. For example, if you submit your application on October 15, your coverage begins January 1. During these three months, you have no public coverage at all.
This is why you must purchase temporary private health insurance (typically through Blue Cross, Desjardins, or Manulife) to cover this gap. Costs range from $100 to $200 CAD per month per person. Do not skip this step: a single emergency room visit in Montreal can cost several thousand dollars without insurance.
Required documents for registration
To submit your application, prepare the following documents:
• Birth certificate or passport;
• Permanent resident card (or confirmation of permanent residence);
• Proof of residence in Quebec: lease, electricity bill, or bank statement with a Quebec address;
• Health insurance card from another province (if transferring from Ontario, for example);
• The completed "Application for registration to the health insurance plan," available on the RAMQ website or at your local CLSC.
You can submit your application online through the Quebec portal, by mail, or in person at your local CLSC. Processing typically takes 4 to 8 weeks. You will receive your health card by mail at your Quebec address.
Finding a family doctor
Having a RAMQ card does not mean you have a family doctor. In Quebec, roughly 20% of residents do not have a family doctor. To join the waiting list, use the Guichet d'accès à la première ligne (GAP) through the ClicSanté Québec website. You fill out a questionnaire about your health condition and location, and you will be matched with an available doctor.
In the meantime, for urgent non-life-threatening needs, you can visit your local CLSC walk-in clinic. For life-threatening emergencies, call 911. For non-urgent medical advice, the 811 Info-Santé line connects you to a nurse 24/7.
What RAMQ covers (and what it doesn't)
Covered: family doctor visits, specialists, hospitalization, lab tests, X-rays, vaccinations, surgeries. For prescription drugs, RAMQ offers a public plan for people not covered by a private plan, with an annual deductible of about $25 and 65% coinsurance.
Not covered: routine dental care, eyeglasses and contact lenses, private physiotherapy, private psychotherapy, chiropractic care, complementary and alternative medicine, care outside Canada (except limited agreements). This is why many families purchase supplementary private insurance (typically $50-$100 per month per adult).
Practical tips to speed up the process
Register for RAMQ during your first week in Montreal, not after finding permanent housing. A temporary address (hotel, student residence, or a friend's place) is sufficient to file the application. Update your address online on the RAMQ website as soon as you move. Keep your health insurance number safe — you will need it at the doctor's office, the pharmacy, when opening a bank account, and when enrolling your children in school.
